• Carbimazole, orally 40 mg total/day (or methimazole 30 mg) until euthyroid (usually
4-6 weeks); then either titrate ('titration regimen') by decrements initially of 10 mg every 4-6 weeks to a maintenance dose of 5-10 mg daily; or continue ('block-replace regimen') 40 mg once daily and add levothyroxine 75-125 micrograms/ day, with monitoring of free T4 and TSH.
• Propylthiouracil, orally, 600 mg total/day until euthyroid: maintenance 50-100 mg total/day. Much higher doses (up to 2.4 g/day) with frequent administration are used for thyroid storm.
