Multiple myeloma is, with rare exceptions, a systemic disease. The neoplastic plasma cells cause various organ dysfunctions through tumor cell invasion and, more importantly, through the pathologic effects of the monoclonal immunoglobulins (Ig) and/or free Ig light chains. In addition, myeloma cells secrete, and stimulate normal stromal cells in the bone marrow microenvironment to secrete, a host of cytokines that mediate various biologic effects. Thus, patients with multiple myeloma may exhibit an array of clinical manifestations.
